He is Founder and CEO of Florida Spine & Pain Specialists, a multi-location interventional pain practice in the Tampa Bay area, and serves as Program Director of its Interventional Spine Fellowship. He also directs a Multidisciplinary Pain Medicine Fellowship affiliated with HCA Florida Blake Hospital and the USF Morsani College of Medicine, and is Medical Director of West Florida Surgical Suites.
Dr. Azeem earned his undergraduate degree at James Madison University and his medical degree from Ross University School of Medicine. He is board certified in Pain Medicine and in Physical Medicine & Rehabilitation by the American Board of Medical Specialties, and is a Fellow of the American Academy of Physical Medicine and Rehabilitation (FAAPMR).
Dr. Azeem’s clinical practice centers on minimally invasive, non-opioid approaches to spine and pain conditions, including radiofrequency ablation, spinal cord stimulation, and sacroiliac joint fusion; he has described building one of the region’s first dedicated non-opioid interventional pain clinics. His research and guideline work is closely tied to ASPN, including co-authorship of the society’s evidence-based clinical guidelines on interventional treatments for low back pain and on implantable peripheral nerve stimulation for chronic pain, both published in the Journal of Pain Research. He has also published on outcomes of minimally invasive posterior sacroiliac joint fusion devices.
Beyond his ASPN Executive Board service, Dr. Azeem is President of the Florida Society of Pain & Neuroscience and holds board positions with several other pain-medicine organizations.
